Acceptable Form of Identification - To complete a notarization, Michigan law requires that I verify your identity using satisfactory evidence. The name on your ID should reasonably match the name on the document you are signing.

Common Acceptable Forms of ID

✔️ Michigan Driver's License

✔️ Driver's License issued by another U.S. state

✔️ Michigan State Identification Card

✔️ State-issued Identification Card from another U.S. state

✔️ U.S. Passport or U.S. Passport Card

✔️ Foreign Passport (if it is current or otherwise acceptable under Michigan law and allows me to reasonably identify you)

✔️ U.S. Military Identification Card

✔️ Federally Recognized Tribal Identification Card

Your ID Should Be:

• Current or otherwise acceptable under Michigan law

• Government-issued

• Contain your photograph

• Include your signature (when applicable)

• Contain identifying information such as your name and date of birth or physical description

Please Note

I may be unable to proceed if:

• Your ID cannot be verified.

• The ID appears altered, damaged, or fraudulent.

• The photograph does not reasonably match the signer.

• The name on the document cannot reasonably be connected to the name on your identification.

• You do not personally appear before me.

Don't Have Acceptable Identification?

Please contact me before your appointment. In some situations, Michigan law allows identity to be established through other legally acceptable means, depending on the circumstances.

Explain:

• Do not sign before the appointment.

• Complete blanks before arrival when possible.

• Bring all pages.

• Bring required witnesses.

  • In some cases, I can provide witnesses for an additional fee.​
